This form is for reporting possible breaches of law, or unjust or unethical behavior (hereinafter “incidents”) by the company or an individual. Reporting an incident is done in privacy, and information such as your name, the details of the report, and the fact that you submitted a report will not be disclosed. Whistleblowers will not be subject to any disadvantage for having reported an incident. All reports received will be investigated, and in doing so, we may ask you to participate in an interview to provide more information.
